This information is in addition to the information you received before your procedure day. (This may include the client’s medical record and the information provided in consultation and on 6emicro.com). 

This information allows you to decide whether or not to give knowledgeable consent to the Scalp Micro-pigmentation procedure. Therefore, please ensure you have read and understood this form and the information provided during your consultation. 

1. The Scalp Micro-pigmentation procedure is elective, and no medical reason requires a client to proceed with the treatment.

2. The Scalp Micro-pigmentation procedure is a form of tattoo. It uses a pigmentation process to apply pigment just below the skin in the dermis in a similar manner to traditional tattoos. Like traditional tattoos, the SMP ink/pressions can be challenging to remove. 6e Micro can not guarantee the success of any removal procedure. 

3. The pigments used in the Scalp Micro-pigmentation procedure are made with natural ingredients (carbon and iron oxide).

4. Clients may experience discomfort (possibly pain in rare cases) during the Scalp Micro-pigmentation procedure.

5. As with any tattoo procedure, there is a risk of infection, although severe conditions are rare.

6. Clients with a history of skin cancer, sun-damaged skin, or any other relevant medical condition should consult their PGP/Physician before undergoing the Scalp Micro-pigmentation procedure. In these circumstances, we may require a signed certificate from the client’s PGP before starting the SMP process.

7. To obtain the best results, the client must comply with our after-care advice described in our literature and attend any follow-up appointments advised by our Artists.

8. If clients tamper with the Scalp Micro-pigmentation procedure while undergoing treatment sessions (including failure to return for necessary follow-up sessions as outlined initially), this will void the 6e Micro guarantee against potential fading. 

Cancellations, Payments, and Complaints

i. We will notify you of the time and date of your procedure appointment. On receipt of this notification, please confirm your attendance. Failure to do so may result in reallocating the appointment time to another client. All Artists of 6e Micro are trained and approved by the company’s procedure protocols and are licensed and insured to perform Scalp Micro-pigmentation. If you cannot attend your appointment, we require 48 hours notice to reschedule. Any cancellations or reschedules within 48 hours will result in a $250 rebooking fee + procedure payment in full to book into the schedule. Please arrive for your procedure at least 15 minutes beforehand so we can take care of the necessary paperwork. Late arrival may result in reduced procedure time or forfeiting of the appointment. 6e Micro will attempt to ensure that your procedure runs on time. However, for reasons beyond our control, we may need to cancel or postpone your appointment at short notice. In this unlikely event, we will try to contact you in advance. We do not offer compensation for postponed appointments. Before your appointment, we will inform you of any preparations required before your procedure.

ii. Deposits – 6e Micro may request a non-refundable deposit to secure your appointment. 6e Micro will deduct this deposit from your procedure cost. The non-refundable deposit will be used as a cancellation charge if you cancel or fail to arrive for your procedure. Should you change your appointment with 48 hours notice, 6e Micro will allocate the deposit to the new appointment. Any cancellations or reschedules within 48 hours will result in a $250 rebooking fee + procedure payment in full to book into the schedule. Payments – 6e Micro will accept the following forms of payment: A. Cash B. Debit/Credit Cards (*3.3% additional fee). The quote provided during your consultation is valid for six months.

iii. Procedure Suitability – We assess whether the procedure is suitable for you or likely to be successful before any procedure. If not, we will explain why you are not a good candidate for the SMP.

iv. Liability – 6e Micro will not be liable in contract, tort, or otherwise for any economic loss (including, without limitation, loss of profit) or any other special, indirect, or consequential loss or damage arising out of or in connection with, its provisions of any goods and or services to the client. The client must provide 6e Micro with all relevant medical details before each procedure. 6e Micro will not be liable for damages due to the client’s failure to disclose such information. The client agrees to comply with all instructions and or recommendations given to them by or on behalf of 6e Micro regarding the care of a treated area. Nothing in these business terms shall exclude or limit 6e Micro’s liability for death or personal injury resulting from 6e Micro’s negligence. 6e Micro provides a satisfaction guarantee against unforeseen fading and or lightening of color. This guarantee lasts 12 months (1 year) from the initial procedure date. This guarantee is effective only after the client has completed three sessions. (This is the number of sessions initially quoted during the consultation). Any alterations to the agreed style, including repositioning of hairline, increase of density, darker color, and other fine-tuning, will not be covered under the guarantee and are chargeable and will incur an additional fee. Should laser removal be necessary, the client will be responsible for the costs incurred.

v. Acknowledgements – I confirm that I have read and understood the information above and any documents provided. In particular, I acknowledge that the Scalp Micro-pigmentation procedure is a permanent change to my appearance and that there is no guarantee that the ink/pressions can be changed or removed later. I understand the efficiency of the Scalp Micro-pigmentation procedure varies from individual to individual. I comprehend that a few clients may fail to respond fully to the treatment, and I, as an individual, may not reply. The Scalp Micro-pigmentation procedure will require multiple sessions (minimum 2) due to the different skin types and hair tones. Some clients may need more than one course of treatment. I understand that Scalp Micro-pigmentation requires some maintenance. I understand that the procedure I receive will be appropriate for my specific needs and performed by a certified and trained Artist of 6e Micro. I understand that post-procedure, the skin will be red, and there can be a small amount of swelling, which is normal. Blistering or crusting is not common but can occur and can take weeks to diminish. I understand that if this happens, I will contact 6e Micro in the first instance. I will follow any instructions/recommendations given to me by 6e Micro regarding the care of the treated area before, during, and after the procedure. After the procedure, treated areas should not be picked, scratched, or traumatized. Moisturize the scalp as advised in the after-care recommendations. I confirm I have informed a 6e Micro representative of (a.) any skin condition I have on the scalp. (b.) any medical condition or medication I am on may affect my suitability as a candidate for the Scalp Micro-pigmentation procedure. (c.) I will inform 6e Micro if I am actively sun tanning. 6e Micro will not be held liable for any damage that results from my failure to adhere to after-care guidelines. I confirm I have been allowed to discuss the Scalp Micro-pigmentation procedure with the 6e Micro Artist, and I have had the chance to ask questions about this procedure. I confirm that I am over eighteen and have provided proof of my age, if necessary. I assure you that the information I have provided to 6e Micro is true and accurate to the best of my knowledge. I understand that 6e Micro will take photographs of my face and scalp before, during, and after the procedure. 6e Micro will store this information to provide me with the Scalp Micro-pigmentation procedure and its internal records. I understand that 6e Micro reserves the right to use these photos on, but not limited to, the company’s social media channels, website, and marketing materials. You agree and opt-in to receive SMS text messages from 6e Micro. I consent to the storage and processing of my data (including any photographs) by 6e Micro to provide me with the Scalp Micro-pigmentation procedure.
